Please select which payment installment you are paying. It is the parent/guardian responsibility to make each payment on time in 5 Star. For each payment installment return to this 5 Star Item to make each payment. The payment plan consists of 3 payments of $60 each for a total of $180. Payment 1 is due by December 31, 2025; Payment 2 is due by January 31, 2026, Payment 3 is due by February 28, 2026. This Payment Plan is only for the Bundle Ticket Option Only (Dance and Disneyland). If you are not purchasing the Bundle please see the Pay in Full item in 5 Star to purchase tickets. The 8th Grade Dance is Friday, 5/29 from 6:30pm – 8:30pm. In order to attend the dance, the 8th Grade Dance Permission Slip must be turned in by Friday, 3/27 at 4pm. Meal choice & beverage requests are not guaranteed. The 8th Grade Disneyland Field Trip is Tues., 6/2 from 9am – 9:45pm. Students must be transported by bus from DJAMS to Disneyland. Students must ride the bus back to DJAMS. In order to go to Disneyland, the 8th Grade Disneyland Permission Slip and Medical Waiver Form must be turned in by Friday, 3/27 at 4pm. Permission slips, medical waiver form and guidelines can be found in the 8th Grade End of Year Activities Packet linked below. To avoid ASB and DJAMS having to pay the cost of credit card processing and other fees, a convenience charge has been added to the order subtotal. If you do not wish to pay this fee, please discontinue your order online and contact Larissa Martus, DJAMS ASB Advisor at Lmmartus@capousd.org with instructions on how to proceed.
